At Synergy Family Services, our Community Health Workers (CHWs) serve as trusted, culturally competent bridges between individuals and the systems that support them. We work alongside therapists, case managers, and healthcare providers to reinforce mental health treatment goals, reduce barriers to wellness, and empower clients in their daily lives.

What We Do

Our CHWs are trained professionals who provide non-clinical, preventative, and supportive services.

They assist Medi-Cal eligible clients with:

  • Emotional regulation coaching & daily stress management
  • Health education & life skills training
  • Goal setting & accountability
  • Crisis navigation & resource linkage
  • Accessing medical, mental health, food, housing, or benefit systems
  • Parenting support & family communication strategies
  • One-on-one or small group skill-building

CHWs do not provide therapy, but their work can enhance clinical progress by increasing consistency, connection, and follow-through between sessions.

Who Can Benefit

Our CHW Services are ideal for Medi-Cal clients who:

  • Need extra support implementing therapeutic strategies
  • Struggle with daily functioning or system navigation
  • Are looking for support while on a waiting list
  • Face barriers to stability, care access, or wellness follow-through
  • Could benefit from frequent, strengths-based check-ins
  • Are not yet ready for therapy, but need engagement and support

How to Refer

To refer a client for CHW services:

We will contact the client within 3 business days to begin services.
